If you are under 18 years old, we need your parents or guardians' formal permission to activate the course you are buying.

Print it and ask your parents or guardians to complete it and confirm that they accept your training at ATO Avioner.
The completed consent form must bear notarized signatures of parents or guardians.
Upload a copy of the signed "Parental consent" to your account at ATO Avioner during registration or later - to Listener's menu, and send the original paper document to the address of the ATO Avioner School by post.
We can activate your training when you have uploaded to the system the copy of the "Parental consent" and the copy of your payment confirmation.
We expect that you will send the original "Parent's consent" to us within 7 days of registering your account here.
We reserve the right to block your training if we do not receive the "Parental consent" within 10 days of the date of your course activation.
In such a situation, the fee paid for the started training (see point 7) is not refundable.

Attention!

If you are a citizen of a country other than those of the European Union, and you want to take part in the training online from the territory of Poland, then according to Polish regulations we must obtain the permission of the Polish aviation authority for your training at ATO Avioner.
Please complete the following procedure to apply for the permission:

Print it and complete points: 3, 4 and 9, then sign in the fields "Date, Candidate signature" on page 2, 3 and 4
Enclose a notarized copy of the document of your residence permit in Poland and a copy of your Passport’s page containing the photo
Send the original documents by post to the following address:
